We are seeking compassionate, clinically competent Registered Nurses to deliver safe, person-centred care across a range of settings. You will assess, plan, implement and evaluate care; act as a patient advocate; and collaborate with multidisciplinary teams to achieve the best outcomes. This role is suitable for nurses with experience or interest in adult, paediatric, community, or home-based care. Key responsibilities are:
- Undertake holistic assessments and develop individualised care plans in partnership with patients and families/carers.
- Deliver evidence-based nursing interventions, medications and treatments in line with policies, protocols and care plans.
- Monitor, interpret and escalate changes in condition; respond to deterioration and safeguarding concerns appropriately.
- Maintain accurate, timely and confidential clinical records and documentation.
- Coordinate care with the multidisciplinary team; contribute to handovers, referrals and discharge planning.
- Provide education and reassurance to patients and families, promoting self-management and independence.
- Make sound clinical judgements within scope of practice; seek support and escalate when required.
- Support infection prevention and control, medicines management and safe use of equipment.
- Participate in audits, quality improvement and incident reporting to enhance safety and outcomes.
- For community/homecare shifts: plan visits, deliver care in the home, and update digital records while mobile.
